Summary

The MDS Coordinator (LPN or RN) plays a critical role in ensuring that patients receive adequate care by managing the Resident Assessment Instrument according to regulations. This position serves as an in-house case manager, coordinating care with medical professionals, families, and facility staff. Ideal candidates are licensed nurses with experience in both clinical settings and long-term care, bringing expertise in assessment and coordination.

Responsibilities

• Complete the Resident Assessment Instrument per federal and state regulations and company policies.
• Act as an in-house case manager, coordinating services with physicians, families, third-party payers, and facility staff.

Qualifications

• Licensed Practical Nurse or Registered Nurse with a current, active license in the state of practice.
• Must hold and maintain a current CPR certification.
• Minimum two (2) years of clinical experience in a healthcare setting.
• Minimum of one (1) year of experience in a long-term care setting.
• Prior experience as an MDS coordinator with demonstrated expertise.
• Must be capable of maintaining regular attendance.
