Grand Valley Health and Rehabilitation, a long-term skilled nursing facility located in Martinsville, IN, is committed to residents and employees. Grand Valley Health and Rehabilitation provides the utmost quality of care to residents by selecting the best qualified MDS Coordinator to work on our team.

Grand Valley Health and Rehabilitation is looking for a licensed LPN or RN to work as an MDS Coordinator. Candidates should have received on-the-job training or completed an MDS-training program. We are also willing to train quick learners. Applicants must be organized, detail-oriented, have great assessment skills, and be able to work independently. This is a Monday through Friday day shift position.

Job Duties

  1. MDS scheduling and completion for OBRA/PPS/Managed Care utilizing RAI guidelines.
  2. Assist with care plan scheduling and completion with the interdisciplinary team.
  3. Assign and update ICD-10 codes based on physician diagnosis on admission and with each MDS.
  4. Transmit OBRA/PPS MDS Assessments to CMS per Federal Guidelines.
  5. Completion and Certifications/Re-certifications when a resident is receiving Medicare Part A Benefit.
  6. Oversee and monitor MDS documentation and charting requirements needed to support services provided.
  7. Educate staff on MDS processes as needed.
  8. Assist with data collection for audits including but not limited to State, Medicare, and Managed Care.
  9. Participate in facility and company meetings per policy.
  10. Become proficient with and apply RAI rules/regulations.
  11. Become familiar with QMs and assist the team in monitoring.

Job Requirements:

  1. Licensed LPN or RN in the state of Indiana.
  2. Applicants should be organized, detail-oriented, have great assessment and communication skills, and be able to work independently and as part of a team.
  3. Applicants should be ambitious, inquisitive, and eager to learn.
