Villa Healthcare is seeking an experienced MDS Coordinator LPN to lead the development and implementation of resident care plans in accordance with state and federal regulations. The role emphasizes coordinating MDS assessments, timely documentation, and collaboration with care teams to optimize patient care and reimbursement.

The ideal candidate holds an LPN license in the state and has SNF/MDS experience; proficiency with Microsoft Office and strong communication are essential.

Qualifications

• Familiarity with the MDS assessment cycle and billing requirements.
• Excellent written and verbal communication skills.
• Proficiency with common software including Word and email.

Responsabilités

• Coordinate completion of each resident’s MDS per State and Federal requirements.
• Ensure timely and accurate MDS scoring and documentation.
• Train CNAs and nurses on required clinical record documentation.
