Ojai Health & Rehabilitation is seeking a licensed LVN/LPN to coordinate and complete MDS assessments, ensuring documentation accuracy for care planning and reimbursement. You will work with a wide range of care teams to develop individualized plans and stay compliant with CMS, state, and facility guidelines.

The role requires knowledge of MDS 3.0/RAI, strong organization, and experience with electronic health records, with a focus on quality outcomes for residents in a skilled nursing setting.

Qualifications

• Current LVN/LPN license in the state of practice.
• Preference for experience in long-term care or skilled nursing facilities.
• Knowledge of MDS 3.0, RAI process, and Medicare/Medicaid reimbursement guidelines.
• Strong organizational skills with attention to detail and accuracy.
• Ability to work collaboratively with interdisciplinary teams.
• Proficiency in electronic health records (EHR) systems.

Responsabilités

• Coordinate and complete MDS assessments and CAAs within timelines.
• Ensure accuracy of resident documentation to support care planning and reimbursement.
• Maintain CMS/state/facility compliance and participate in QA initiatives.
• Collaborate with interdisciplinary team to develop individualized care plans.
• Monitor resident progress and update care plans accordingly.
• Assist with audits and surveys by providing accurate documentation.
• Stay current with regulatory changes impacting MDS and documentation.
