Job summary

Riverside Health and Rehabilitation is seeking a qualified RN to coordinate patient assessments and care plans in a collaborative, patient-centered environment. The role requires an active Georgia RN license and a nursing degree, with strong computer and communication skills to liaise across departments and families.

Qualifications

• An Nursing Degree from an accredited college or university, or is a graduate of an approved RN program.
• Active RN License to practice in the State of Georgia

Responsibilities

• Facilitates communication between nursing center departments and patient/family members to provide an effective assessment/care plan process.
• Establishes open lines of communication with interdisciplinary team members to provide a comprehensive and holistic patient care plan.
• Maintains schedule of assessment/care plan meetings.
• Coordinates development of a care plan for each patient with problems/needs, interventions and measurable goals.
• Ensures that assessments and care plans are completed in a timeframe compliant with current regulations.
