Job summary

Lakeland Rehabilitation and Healthcare Center in Indiana seeks an experienced MDS Nurse to ensure accurate assessment coding and up-to-date care plans for all residents. The role collaborates with the RN Coordinator to establish reimbursement rates and quality metrics for public reporting, and ensures care records reflect the plan of care and care rendered.

The MDS Nurse will lead ongoing MDS coordination, review ICD-10 coding, and monitor records for consistency, while providing staff training

Qualifications

• Must hold a current Indiana nursing license (RN or LPN).
• Experience with MDS, EMR, and care planning.
• Strong organizational, computer, and interpersonal skills.
• Ability to initiate, complete, and update care plans with MDS assessments.
• Ability to conduct in-service training of nursing staff on care plans and documentation.

Responsibilities

• Coordinate completion of the MDS for initial admission, readmission, quarterly, annual, significant changes, and PPS/insurance needs.
• Ensure timely completion of each MDS section with proper department input and EMR sign-off.
• Review ICD-10 diagnosis coding and sequencing at admissions and with each MDS assessment; complete Physician Query as needed.
• Conduct daily review of clinical records to capture highest level of care for OBRA and PPS assessments.
• Lead daily MDS-therapy and weekly Utilization Review meetings; participate in pre-billing review of claims.
