Job summary

Jobot is seeking a qualified MDS Coordinator to join our long-term care facility team in the United States. The role involves coordinating the MDS assessment process, collaborating with nursing, therapy, social services, diet, and other disciplines to develop and maintain resident care plans, and supporting reimbursement processes under CMS and PDPM.

Qualifications

• Active nursing license or ability to obtain one.
• Experience with MDS in skilled nursing or long-term care.
• Strong knowledge of RAI, MDS 3.0, Medicare/Medicaid guidelines, and PDPM.
• Strong organizational skills and attention to detail.
• Ability to manage deadlines and multiple assessments.
• Excellent communication and interdisciplinary collaboration skills.

Responsibilities

• Coordinate and complete MDS assessments accurately and within required timeframes.
• Manage the RAI/MDS process in accordance with CMS and state regulations.
• Develop and maintain individualized resident care plans in collaboration with the interdisciplinary team.
• Monitor assessment schedules and ensure timely completion of required documentation.
• Review clinical documentation for accuracy, completeness, and appropriate reimbursement.
• Support Medicare and Medicaid reimbursement processes, including PDPM.
• Participate in care plan meetings and communicate with residents, families, and members of the care team.
• Monitor changes in resident condition and ensure assessments and care plans are updated appropriately.
• Maintain compliance with federal, state, and facility standards.
• Assist with audits, surveys, and quality improvement initiatives.
• Serve as a resource to clinical staff regarding MDS documentation and regulatory requirements.
