Job summary

Jobot is seeking an MDS Coordinator for a long-term care facility in the United States. The role oversees the MDS assessment process and collaborates with nursing, therapy, social services, and dietary teams to develop resident care plans.

The position requires MDS experience, strong regulatory knowledge, and excellent organizational and communication skills. A nursing license is preferred, with emphasis on accurate documentation and regulatory compliance.

Qualifications

• Active nursing license or ability to obtain one.
• Experience with MDS in skilled nursing or long-term care.
• Strong understanding of the RAI process, MDS 3.0, Medicare/Medicaid guidelines, and PDPM.
• Excellent organizational skills and attention to detail.
• Ability to manage deadlines and multiple assessments concurrently.

Responsibilities

• Coordinate and complete MDS assessments accurately and within required timeframes.
• Manage the RAI/MDS process in accordance with CMS and state regulations.
• Develop and maintain individualized resident care plans in collaboration with the interdisciplinary team.
• Monitor assessment schedules and ensure timely completion of required documentation.
• Review clinical documentation for accuracy, completeness, and appropriate reimbursement.
• Support Medicare and Medicaid reimbursement processes, including PDPM.
• Participate in care plan meetings and communicate with residents, families, and care team members.
• Monitor changes in resident condition and ensure assessments and care plans are updated accordingly.
• Maintain compliance with federal, state, and facility standards.
• Assist with audits, surveys, and quality improvement initiatives.
• Serve as a resource to clinical staff regarding MDS documentation and regulatory requirements.
