Job summary

Creative Solutions In HealthCare seeks a dedicated MDS Coordinator to ensure accurate assessments, regulatory compliance, and optimal care planning for residents. You will oversee admissions and multiple assessments, coordinate care plans, and lead interdisciplinary meetings to enhance resident care.

The role requires a current Nursing License, geriatric nursing experience, and strong documentation/communication skills to ensure compliance with state and federal guidelines.

Qualifications

• Must hold a current Nursing License in good standing.
• Experience in geriatric nursing and resident-centered care.
• Excellent assessment, documentation, and communication skills.
• Understanding of nursing home regulations and compliance requirements.

Responsibilities

• Oversee admission, annual, quarterly, Medicare, and change-of-condition assessments and communicate with nursing and other departments.
• Follow Federal and State guidelines for comprehensive assessments (MDS).
• Review MDS and care plan files for accuracy and thoroughness.
• Maintain expertise in MDS software for correct data entry and reporting.
• Attend and coordinate Medicaid, ITTM, and care plan meetings for team communication.
• Update care plans based on current assessments and resident status.
• Generate and distribute Quality Indicators Reports to leadership.
• Ensure timely transmission of MDS data per standards.
• Stay current with industry standards and best practices.
