Qualifications

• Minimum 2 years in a healthcare setting.
• Strong understanding of MDS processes and CMS guidelines.
• Excellent communication and interpersonal skills.
• Familiarity with electronic medical records and coding systems.

Responsibilities

• Complete and coordinate all MDS assessments in CMS guidelines.
• Ensure accurate and timely submission of MDS assessments.
• Participate in interdisciplinary meetings related to resident assessments.
• Care planning: collaborate with nursing/therapy to develop individualized plans.
• Maintain compliance with federal/state/facility regulations for MDS and care planning.
• Prepare for surveys and audits with complete documentation.
• Educate nursing staff on proper documentation practices.
• Monitor case mix index (CMI) and reimbursement trends; support coding accuracy.
• Report quality measures and outcomes; liaise with nursing, therapy, and billing.
