• Maintain compliance with federal/state/facility regulations for MDS and care planning.
• Prepare for surveys and audits with complete documentation.
• Educate nursing staff on proper documentation practices.
• Monitor case mix index (CMI) and reimbursement trends; support coding accuracy.
• Report quality measures and outcomes; liaise with nursing, therapy, and billing.

Responsibilities:

• Complete and coordinate all MDS assessments (admission, quarterly, annual, significant change) in compliance with CMS guidelines
• Ensure accurate and timely submission of MDS assessments
• Participate in interdisciplinary team meetings related to resident assessments
• Care Planning:
  • Collaborate with nursing, therapy, and other departments to develop individualized care plans
  • Ensure care plans reflect residents' current needs, goals, and preferences
  • Update care plans based on assessment findings and changes in condition
• Regulatory Compliance:
  • Maintain compliance with all federal, state, and facility regulations related to MDS and care planning
  • Prepare for surveys and audits by ensuring documentation is complete and accurate
  • Stay updated on CMS/RUG/PDPM guidelines and changes
• Clinical Documentation:
  • Review medical records to ensure supporting documentation aligns with MDS coding
  • Educate nursing staff on proper documentation practices
  • Identify and correct discrepancies in clinical documentation
• Reimbursement & Quality Measures:
  • Monitor case mix index (CMI) and reimbursement trends
  • Assist in maximizing reimbursement through accurate coding
  • Track and report quality measures and outcomes
• Collaboration & Communication:
  • Serve as a liaison between nursing, therapy, and billing departments
  • Communicate effectively with physicians and other healthcare professionals
  • Participate in Quality Assurance and Performance Improvement (QAPI) activities

Requirements:

• Minimum 2 years of experience in a healthcare setting
