Responsibilities

• Coordinate and complete MDS assessments per schedules and guidelines.
• Review resident records to support accurate MDS completion.
• Conduct resident interviews and gather information from residents, families, and staff.
• Collaborate with the interdisciplinary team to obtain complete MDS data.
• Identify changes in resident condition requiring assessment updates.
• Maintain CMS and state regulatory knowledge related to MDS and care planning.
