Job summary

Fayette Health and Rehabilitation Center is seeking an MDS Coordinator in Maryland to coordinate and complete MDS assessments for residents in accordance with CMS and state requirements.

You will work closely with nursing, therapy, social services, dietary, and other team members to ensure accurate assessments, timely documentation, and compliant care planning. Strong MDS knowledge and an active Maryland RN license are essential.

Qualifications

• Current, active Maryland RN license.
• Experience in skilled nursing or long-term care preferred.
• MDS experience strongly preferred.
• Knowledge of the MDS/RAI process and long-term care regulatory requirements.
• Strong clinical assessment and documentation skills.
• Ability to organize and prioritize multiple assessments and deadlines.
• Excellent communication and teamwork abilities.
• Proficiency with EMR and standard computer applications.

Responsibilities

• Coordinate and complete MDS assessments according to required schedules and regulatory guidelines.
• Review resident medical records, clinical documentation, nursing documentation, therapy records, and other information to support accurate MDS completion.
• Conduct resident interviews and gather information from residents, families, staff, and others as needed.
• Coordinate with interdisciplinary team to obtain accurate information for MDS assessments.
• Identify changes in resident condition requiring additional assessment or care plan intervention.
• Participate in interdisciplinary care plan meetings and maintain resident care plans.
• Monitor MDS schedules, deadlines, and documentation to ensure timely completion.
• Review documentation for accuracy and follow up with departments for clarification.
