Job summary

The Blossoms at Breckenridge Rehab & Nursing Center is seeking a qualified MDS Coordinator/Nurse (RN or LPN) to conduct federally mandated assessments, compile comprehensive MDS data, and help determine care plans and reimbursement across payer sources under CMS RUG-IV guidelines.

Responsibilities include completing OBRA, PPS, and MDS forms, coordinating care plans with the interdisciplinary team, scheduling care plan conferences, maintaining OBRA calendars, and submitting data to CMS

Qualifications

• Must be a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
• Optional: MDS Certification - American Association of Nurse Assessment Coordinators (AANAC).

Responsibilities

• Conduct federally mandated MDS assessments for residents and assemble data to support care planning and reimbursement.
• Coordinate OBRA, PPS and Managed Care MDS and related forms.
• Develop and update nursing care plans with the interdisciplinary team.
• Schedule Care Plan Conferences within 21 days of admission and every 90 days thereafter.
• Maintain the OBRA calendar and MDS schedules; ensure timely submissions to CMS.
