Job summary

Diversicare is seeking an MDS Coordinator (RN) to join our skilled nursing team. You will coordinate the RAI/MDS process, work with an interdisciplinary team, and ensure timely, accurate coding and reporting in accordance with state and federal regulations.

The ideal candidate will have an RN license, experience with MDS 3.0, and familiarity with long-term care. Strong data entry skills and the ability to lead care coordination meetings are essential.

Qualifications

• Two years of MDS experience preferred, but not required.
• Must hold current registered nursing license in the state of employment.
• Experience in Long Term Care preferred.
• Computer skills such as Data Entry and Word processing required.
• Knowledgeable of nursing practices, regulations and guidelines for skilled nursing facilities.

Responsibilities

• Coordinate the RAI process.
• Collaborate with the interdisciplinary team to assess patient needs.
• Lead Care Coordination meetings and ensure timely MDS assessments.
• Code MDS accurately using medical records, observations and interviews.
• Maintain MDS schedule tracking and validate state/federal reports.
• Coordinate and complete documentation submissions per guidelines.
• Ensure Medicare/Medicaid guidelines are completed accurately and timely.
