Job summary

Gallup Nursing & Rehabilitation in Gallup, NM seeks an experienced MDS Coordinator to manage RAI/MDS 3.0, care plans, and compliance from admission to discharge. You will collaborate with the interdisciplinary team, ensure timely data entry, and maintain accurate documentation in line with federal, state, and facility guidelines.

This role reports to the Director of Nursing and supports reimbursement and quality initiatives.

Qualifications

• Knowledge of MDS 3.0 and RAI guidelines.
• Ability to coordinate care plans with an interdisciplinary team.
• Strong understanding of Federal/State regulations for SNFs.
• Excellent documentation and data-entry accuracy.

Responsibilities

• Coordinate MDS assessments and care plan reviews for residents.
• Ensure timely and accurate completion of RAI and care management processes.
• Maintain MDS status and data entry in computerized systems.
• Monitor documentation to support MDS coding and compliance.
• Attend interdisciplinary meetings and communicate changes to staff.
