Job summary

The Palazzo Senior Living in Phoenix is hiring a Full-Time MDS Coordinator to coordinate timely patient assessments, ensure CMS and regulatory compliance, and educate care teams. A current RN license and MDS experience are required.

You will work closely with the DON, participate in PPS and billing processes, and help maintain high standards of patient care and accuracy in documentation across departments.

Qualifications

• Current state RN license required.
• Two years’ nursing and/or one year of MDS experience required.
• Outstanding communication (verbal and written) skills.
• Superior organizational skills and attention to detail.
• Takes pride in serving others and completing duties on time.

Responsibilities

• Coordinates and assists with completion and submission of accurate, timely patient assessments.
• Reports changes in a patient’s condition to the Director of Nursing (DON).
• Educates direct care associates on patient documentation and regulations.
• Ensures comprehensive and accurate care plans.
• Reviews CMS reports and coordinates with Billing to ensure timely submissions.
• Attends Daily PPS Meetings and other required meetings.
• Performs staff nurse functions when necessary and models professional conduct.
