Job summary

Casa Maria Healthcare is seeking a MDS Coordinator to lead timely and accurate completion of the MDS process and care management from admission to discharge in compliance with policy and federal/state guidelines. Under supervision, coordinates information systems operations for the clinical department.

The role requires RN/LVN licensure with at least 2 years in a Skilled Nursing Facility, strong knowledge of MDS 3.0, Case-Mix, Medicare PPS, and OBRA regulations, and careful attention to

Qualifications

• Graduate of an approved RN/LVN program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Thorough knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Understanding of Quality Indicator process, OBRA regulations and MDS.
• Experience with MDS 3.0 is preferred.

Responsibilities

• Works with Interdisciplinary Team to assess resident needs and schedules MDS assessments and care plan reviews.
• Decides on completing or not completing additional MDS assessments based on clinical criteria.
• Assists with coordination and management of daily stand-up meetings and assessment reference dates.
• Complies with federal and state regulations regarding MDS processes.
• Monitors MDS and care plan documentation to ensure accuracy in medical records.
• Maintains current MDS status according to guidelines.
• Ensures accurate data entry of resident information into MDS programs.
• Performs other duties as assigned.
