Palolo Nursing and Rehabilitation is seeking an MDS Coordinator to lead and coordinate MDS assessments for residents, ensuring accuracy and compliance with state and federal regulations. You will work closely with the interdisciplinary team to reflect each resident’s needs in documentation.

The role emphasizes collaboration, care-plan updates, and resident-centered documentation. A RN license and a bachelor’s degree are required, with training available for candidates new to MDS.

Qualifications

• Bachelor’s degree in Nursing or related field required.
• Active RN license.
• MDS experience helpful but training provided.

Responsibilities

• Coordinate and complete MDS assessments with accuracy, integrity, and timeliness.
• Collaborate with nurses, therapists, and other healthcare professionals to gather essential information.
• Ensure compliance with all state and federal MDS regulations.
• Review and update individualized care plans based on assessment findings.
• Communicate assessment outcomes to the care team to support resident-centered planning.
• Participate in quality improvement initiatives related to MDS and care planning.
• Maintain organized, up-to-date records of assessments and care plans.
