Interlochen-Health-and-Rehabilitation-Cente is seeking an MDS Coordinator to oversee resident assessments, ensure regulatory compliance, and maintain accurate MDS documentation. You will lead team meetings, update care plans, and report quality indicators to leadership.

The role requires an active nursing license, geriatric care experience, and strong organizational skills to manage multiple priorities in a collaborative environment.

Qualifications

• Current Nursing License in good standing.
• Experience in geriatric nursing and resident-centered care.
• Strong assessment, documentation, and communication skills.
• Knowledge of nursing home regulations and compliance requirements.
• Ability to manage multiple priorities with accuracy.

Responsibilities

• Coordinate admission, annual, quarterly, Medicare and change-of-condition assessments.
• Ensure regulatory compliance with Federal and State MDS guidelines.
• Monitor MDS documents and care plans for accuracy and organization.
• Utilize MDS software for correct data entry and reporting.
• Lead interdisciplinary meetings for smooth team communication.
• Update care plans based on current assessments and resident status.
• Generate and distribute Quality Indicators Reports to leadership.
• Submit MDS data per reporting standards.
• Stay current with industry standards through professional development.
• Oversee ancillary data reporting to corporate billing.
