Job summary

Beltline-Rehabilitation-Center is seeking an MDS Coordinator to ensure accurate assessments and compliant care planning for residents. You will lead interdisciplinary coordination and manage MDS data entry and reporting, contributing to high-quality resident care.

The role requires a current RN license, geriatric nursing experience, and strong organizational skills. You will work within a team to maintain regulatory standards and drive continuous improvement in care delivery.

Qualifications

• Holds a current Nursing License in good standing.
• Experience in geriatric nursing and resident-centered care.
• Excellent assessment, documentation, and communication skills.
• Understands nursing home regulations and compliance requirements.
• Able to work independently and in a team.
• Able to manage multiple priorities with accuracy and efficiency.

Responsibilities

• Coordinate resident assessments including admission, annual, quarterly, Medicare, and change-of-condition assessments.
• Ensure regulatory compliance with Federal and State MDS guidelines.
• Monitor MDS and care plan documentation for accuracy and organization.
• Maintain proficiency in MDS software for data entry and reporting.
• Lead interdisciplinary meetings to coordinate care planning.
• Update and revise care plans based on current assessments.
• Generate and distribute quality indicators reports to leadership.
• Submit MDS data per reporting standards.
• Stay current with professional development in the field.
• Oversee ancillary data reporting to corporate billing.
