Job summary

Interlochen Health is seeking an experienced MDS Coordinator to ensure accurate resident assessments and robust care planning. You will coordinate admissions, annual and quarterly reviews, and Medicare assessments while maintaining strict regulatory compliance and timely communication with the care team.

The role requires a licensed RN with geriatric experience and strong skills in assessment, documentation, and cross-team collaboration.

Qualifications

Current Nursing License in good standing.
Experience in geriatric nursing and resident-centered care.
Strong assessment, documentation, and communication skills.
Knowledge of nursing home regulations and compliance requirements.

Responsibilities

Coordinate resident assessments including admissions, annual, quarterly, Medicare, and change-of-condition assessments.
Ensure regulatory compliance with Federal and State guidelines for MDS.
Monitor MDS and care plan documentation for accuracy and organization.
Maintain proficiency in MDS software for accurate data entry and reporting.
Lead interdisciplinary meetings for Medicaid, ITTM, and care plan coordination.
Update care plans based on current assessments and resident status.
Generate and distribute Quality Indicators Reports to leadership.
Submit MDS data per reporting standards.
Stay current with industry standards through professional development.
Oversee ancillary data reporting to corporate billing.
