Job summary
Interlochen Health and Rehabilitation Center in Arlington, TX is seeking an experienced MDS Coordinator to oversee resident assessments, ensure regulatory compliance, and coordinate comprehensive care planning for our residents. You will lead interdisciplinary meetings, monitor MDS accuracy, and support quality reporting to leadership.
The ideal candidate holds a current Nursing License, has geriatric nursing experience, and demonstrates strong assessment, documentation, and communication
Qualifications
- RN license in good standing.
- Geriatric nursing experience preferred.
- Strong assessment and documentation skills.
- Excellent communication abilities and teamwork.
- Solid knowledge of federal and state regulations related to MDS.
Responsibilities
- Coordinate resident assessments (admission, annual, quarterly, Medicare, change-of-condition).
- Ensure regulatory compliance for MDS processes.
- Review MDS and care plans for accuracy and completeness.
- Maintain MDS software proficiency for accurate data entry and reporting.
- Lead interdisciplinary meetings (Medicaid, ITTM, care plans).
- Update and revise care plans based on assessments.
