Rockdale Healthcare Center in Conyers, GA seeks an MDS Coordinator to manage the RAI process, ensure timely MDS completion, and lead the interdisciplinary care planning for residents. This role requires strong leadership, detailed documentation, and compliance with federal/state regulations.

The ideal candidate will have an active RN/LVN license, with preferred experience in long-term care and proficiency in PointClickCare and hospital-style record management.

Qualifications

• Must possess a current, unencumbered RN or LVN license in the state.
• RN license preferred; LVN acceptable.
• 2 years’ Long-Term Care experience preferred.
• RAI process experience preferred (2 years).
• Ability to make independent decisions when necessary.
• Demonstrated leadership and organizational skills.
• Strong PCC (PointClickCare) knowledge preferred.
• Computer literacy including Outlook, MS Office, and internet access.

Responsabilités

• Maintain current MDS status for each resident per laws and facility policies.
• Schedule OBRA, Medicare PPS and Medicaid assessments and communicate schedules.
• Monitor resident condition changes and coordinate MDS reassessments and care plans.
• Strategically set ARD with the interdisciplinary team to reflect resident needs.
• Complete MDS and CAAs accurately per procedures.
• Attend daily Casemix, weekly Utilization Review, and monthly Triple Check meetings.
• Maintain assessments for at least 15 months in the active record; transmit MDS data to state.
