Job summary

Topaz Fiscal Services is seeking an MDS Coordinator in Conyers, GA to oversee the Resident Assessment Instrument (RAI) process, complete the MDS accurately and timely, develop the interdisciplinary plan of care, and lead the daily Medicare PPS/Medicaid case-mix meetings.

The role requires an active LVN or RN license, with RN license preferred, and experience in long-term care and RAI processing. Strong leadership, organizational skills, and PCC/MS Office proficiency are valued traits.

Qualifications

• Must possess an active LVN or RN license in the state.
• RN license preferred for this role.
• Experience with MDS/RAI in long-term care is advantageous.

Responsibilities

• Maintain current MDS status for each resident per federal/state laws and facility policy.
• Schedule and coordinate OBRA, Medicare PPS, and Medicaid assessments with relevant departments and families.
• Monitor resident condition changes and trigger MDS re-assessments and care-plan updates.
