Job summary

NurseCare Buckhead seeks a Licensed Registered Nurse to coordinate and facilitate timely RAI and Care Management processes from admission to discharge in accordance with state and federal guidelines. You will educate the clinical team and coordinate information system operations as needed.

Qualifications

• MDS experience is required.
• Knowledge of Medicare processes is preferred.
• CEUs as required by the State.

Responsibilities

• Coordinate and participate in care management meetings with the interdisciplinary team.
• Ensure accurate resident assessments per State and Federal regulations.
• Maintain MOS schedules and data entry across MOS programs.
• Coordinate education related to RAI/Care Management per policy.
• Submit required documentation to State databases and other entities.
