Job summary

Creative Solutions in Healthcare, Inc. is seeking an experienced MDS Coordinator to ensure accurate resident assessments and compliant care planning.

You will manage admissions, annuals, and change-of-condition assessments, coordinating with nursing and multiple departments to maintain quality and regulatory adherence. The role requires strong organization, attention to detail, and proficiency with MDS software.

Qualifications

• RN licensure required and in good standing
• Experience in geriatric nursing and resident-centered care
• Strong assessment, documentation, and communication skills
• Familiarity with nursing home regulations and compliance requirements

Responsibilities

• Coordinate resident assessments: admissions, annual, quarterly, Medicare, and change-of-condition assessments; communicate with nursing and other departments
• Ensure regulatory compliance with Federal and State guidelines for MDS
• Review MDS and care plan files for accuracy and organization
• Maintain expertise in MDS software for correct data entry and reporting
• Lead interdisciplinary meetings (Medicaid, ITTM, care plan) for smooth communication
• Update care plans based on current assessments and resident status
• Generate and distribute Quality Indicators Reports to leadership for care improvement
• Submit MDS data per reporting standards in a timely, accurate manner
• Stay current by attending professional development sessions
• Oversee ancillary data reporting to corporate billing for accuracy
