Creative Solutions in Healthcare, Inc. is seeking an experienced MDS Coordinator in Texas to ensure accurate assessments and compliant care planning for residents. You will oversee admissions and quarterly reviews, maintain MDS accuracy, and support interdisciplinary coordination to improve care outcomes.

The role requires strong organizational skills, reliability, and a collaborative approach to reporting and data management within our facility.

Qualifications

• Holds a current Nursing License in good standing.
• Experience in geriatric nursing and resident-centered care.
• Strong assessment, documentation, and communication skills.
• Understanding of nursing home regulations and compliance requirements.

Responsibilities

• Coordinate Resident Assessments: admission, annual, quarterly, Medicare, and change-of-condition assessments.
• Ensure Regulatory Compliance: follow Federal and State guidelines for MDS.
• Monitor Documentation: review MDS and care plan files for accuracy and organization.
• Utilize MDS Software: maintain data entry and reporting accuracy.
• Lead Interdisciplinary Meetings: coordinate Medicaid, ITTM, and care plan meetings.
• Update Care Plans: revise care plans as needed based on assessments.
• Provide Quality Reporting: generate and distribute Quality Indicators Reports.
• Submit MDS Data: ensure timely transmission per standards.
• Stay Current: attend professional development sessions.
• Oversee Ancillary Data: collect and report ancillary services to billing.
