Ideal candidates hold a current nursing license, have geriatric care experience, and can work independently within a team while handling multiple priorities with precision.

Qualifications

• Holds a current Nursing License in good standing.
• Experience in geriatric nursing and resident-centered care.
• Strong assessment, documentation, and communication skills.
• Understanding of nursing home regulations and compliance requirements.
• Ability to manage multiple priorities with accuracy.

Responsibilities

• Coordinate resident assessments (admission, annual, quarterly, Medicare, change-of-condition).
• Ensure regulatory compliance with Federal and State guidelines for MDS.
• Monitor documentation and care plan files for accuracy and organization.
• Maintain MDS software proficiency for data entry and reporting.
• Lead interdisciplinary meetings and update care plans as needed.
• Generate quality indicators reports for leadership.
• Submit MDS data per reporting standards.
