You will work with nursing and interdisciplinary teams to ensure accurate data entry, timely care planning, and high-quality care reporting. This role demands strong organizational skills and a collaborative approach to improve resident outcomes.

Qualifications

• Holds a current Nursing License in good standing.
• Experience in geriatric nursing and resident-centered care.
• Excellent assessment, documentation, and communication skills.
• Understands nursing home regulations and compliance requirements.
• Works well independently and in a team.
• Manages multiple priorities with accuracy and efficiency.

Responsibilities

• Coordinate Resident Assessments: admissions, annual, quarterly, Medicare, change-of-condition.
• Ensure Regulatory Compliance with Federal and State guidelines for MDS.
• Monitor Documentation: review MDS and care plans for accuracy.
• Utilize MDS Software for accurate data entry and reporting.
• Lead Interdisciplinary Meetings: Medicaid, ITTM, and care plan coordination.
• Update Care Plans based on current assessments and resident status.
• Provide Quality Reporting: generate indicators reports for leadership.
• Submit MDS Data: timely transmission per standards.
• Stay Current: professional development to maintain best practices.
• Oversee Ancillary Data: report to corporate billing for accuracy.
