Job summary

Qualifications

• At least 2 years of experience in a healthcare setting.
• LVN certification or equivalent is required.
• A degree in a healthcare-related field is required.

Responsibilities

• Coordinate the development and maintenance of MDS 3.0 assessments for residents, ensuring timely completion.
• Collaborate with interdisciplinary team to analyze resident data and support care planning.
• Monitor and report resident outcomes to state and federal agencies as required.
• Conduct unit-based assessments and address resident care needs with interventions.
• Maintain relationships with physicians, nurses, and therapists to support care.
• Keep accurate records of assessments, care plans, and progress.
