Avir at Sealy seeks a Licensed RN MDS Coordinator to join our Texas team and champion resident-centered care. You will coordinate MDS assessments and collaborate with an interdisciplinary care team to develop effective plans.

You will ensure regulatory compliance, maintain accurate records, and provide education to staff, residents, and families as part of our commitment to quality outcomes and compassionate service.

Qualifications

• Active RN license in good standing.
• Knowledge of MDS (Minimum Data Set) processes and timelines.
• Experience coordinating care plans with interdisciplinary teams.
• Familiarity with regulatory requirements and documentation standards.

Responsibilities

• Collaborate with interdisciplinary team to develop and implement individualized care plans.
• Conduct comprehensive assessments of residents' needs and identify improvement areas.
• Coordinate and manage MDS assessments ensuring accuracy and timeliness.
• Maintain accurate resident records in compliance with regulatory standards.
• Participate in quality improvement initiatives to enhance resident outcomes.
• Provide education and support to staff, families, and residents.
