Job summary

Avir at Sealy is seeking a licensed RN or LVN MDS Coordinator to join our care-focused team in Sealy, Texas. You will lead MDS assessments, coordinate care planning with interdisciplinary staff, and promote excellence in resident outcomes through meticulous documentation and regulatory compliance.

You will collaborate with care teams, conduct comprehensive resident assessments, and contribute to quality improvement initiatives while supporting families and staff with education and compassion.

Qualifications

• Must hold an active RN or LVN license in Texas.
• Experience coordinating MDS processes and care planning.
• Strong communication and regulatory knowledge.

Responsibilities

• Collaborate with interdisciplinary team to develop and implement care plans and ensure clear communication among caregivers.
• Conduct comprehensive assessments of residents’ needs and propose recommendations for care delivery improvements.
• Coordinate and manage MDS assessments ensuring accuracy and timely submissions.
• Maintain accurate resident records in accordance with regulatory standards and internal policies.
• Participate in quality improvement initiatives to improve resident outcomes and satisfaction.
• Provide education and support to staff, families and residents to foster open communication and compassion.
