Job summary

Signature HealthCARE at Colonial Rehab & Wellness Center in Bardstown, KY is seeking an MDS Candidate who is detail-oriented and patient-focused. The role involves bedside resident assessment, collecting data, and coordinating with families to ensure accurate MDS data capture.

The ideal candidate holds an LPN/LVN license with current state licensure and has at least 3 years of clinical experience, including 1 year in MDS. This position offers growth paths and a comprehensive benefits package.

Qualifications

• Licensed Practical/Vocational Nurse with current state licensure.
• Minimum three years of clinical experience in a health care setting (long-term care preferred).
• Minimum one year of MDS experience.

Responsibilities

• Collect information to complete the MDS from medical records, bedside assessments, and interviews.
• Participate in Daily PPS meetings, weekly Medicare meetings, and month-end meetings to ensure federal billing requirements are met.
• Review Plan of Care at least quarterly and with each Comprehensive Assessment to capture changes.
