SUMMARY: The MDS Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents to ensure appropriate reimbursement for care and services. Conducts continual Minimum Data Set (MDS) reviews to optimize Resource Utilization Group (RUG) categories. Oversees the process and tracking of MDS/PPS documentation and submission, integrating nursing, dietary, social, recreational, restorative, rehabilitation, and physician services to ensure proper assessment and reimbursement.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
  1. Assess and determine the health status and care level of new admissions.
  2. Ensure accurate and timely completion of all MDS assessments, including PPS Medicare, quarterly, annual, and significant change assessments.
  3. Communicate the care level for new residents to all disciplines.
  4. Coordinate interdisciplinary participation in completing the MDS for each new admission within regulatory timeframes, ensuring documentation compliance with federal, state, and medical standards.
  5. Maintain an accurate schedule of all MDS assessments with proper reference dates.
  6. Handle data entry to ensure accurate electronic submission of MDS assessments, verify submissions, perform corrections, and maintain records.
  7. Schedule and conduct resident care conferences, ensuring all MDS reviews are completed beforehand.
  8. Assist disciplines in care plan formulation and revision, ensuring problems are identified, goals are realistic, and interventions are appropriate.
  9. Evaluate and revise resident care plans for comprehensiveness and individualization.
  10. Assess achievement of care goals, and if necessary, re-assess and revise care plans.
  11. Manage level of care changes, notify relevant departments, and process forms accordingly.
QUALIFICATIONS:
  • Registered Nurse with current licensure.
  • Long-term care experience preferred.
  • Proficiency in reading, writing, speaking, and understanding English.
PHYSICAL DEMANDS:
  • Regular sitting, standing, bending, walking; ability to lift/move up to 25 pounds.
  • Sufficient visual and auditory abilities for communication.
  • Moderate noise level environment.
