Tharp Consulting is seeking a dedicated MDS Coordinator to manage the Medicare Resident Assessment Instrument (MDS) and care planning for a senior living community. You will conduct assessments, facilitate resident interviews, and translate findings into individualized care plans to ensure high-quality resident care.

Collaborating with treatment teams, families, and community specialists, you will refine care plans and align stakeholders with resident goals and priorities to deliver

Qualifications

• Strong knowledge of MDS and care planning processes.
• Excellent communication and interpersonal skills for working with residents and families.
• Ability to synthesize assessment information into actionable care plans.
• Detail-oriented and organized with a focus on quality and compliance.
• Collaborative mindset and ability to coordinate across multidisciplinary teams.

Responsabilités

• Coordinate and oversee the MDS assessment and care planning workflow.
• Assess residents’ quality of care and identify care needs.
• Conduct initial resident interviews to support accurate, person-centered assessments.
• Develop individualized care plans based on assessment outcomes.
• Collaborate with treatment teams, caretakers, families, and community specialists to implement and refine care plans.
• Serve as a liaison to align stakeholders around resident goals and care priorities.
