Arcadia LTC in Towanda, IL seeks an MDS Coordinator to ensure accurate Medicare/Medicaid case-mix documentation and timely care plans to optimize PDPM/RUG reimbursement. This in-person role coordinates nursing, dietary, social, and physician services to support comprehensive resident assessments and proper documentation.

The position requires RN or LPN licensure, LTC experience, and restorative certification within six months if not already certified. Excellent English communication is essential.

Qualificações

• Registered Nurse or Licensed Practical Nurse with current licensure.
• Long Term Care experience required.
• Restorative Certification required within six months of hire if not already certified.
• Ability to read, write, speak and understand English.

Responsabilidades

• MDS Assessment and Documentation: Assess health status and level of care for new admissions.
• Ensure accurate and timely completion of all MDS Assessments (PPS Medicare, quarterly, annual, significant change).
• Maintain an accurate schedule of all MDS assessments with proper reference dates.
• Care Planning and Resident Assessment: Complete care plans on admission, quarterly and as needed according to regulatory time frames.
• Care Conferences and Interdisciplinary Coordination: Schedule and conduct resident care conferences and ensure completion of all MDS reviews.
• Restorative Nursing Program Coordination: Oversee that restorative nursing measures are reflected on the care plan.
