Apple Rehab Inc in Hartford, CT is seeking an experienced MDS Care Plan Coordinator to lead the MDS department and coordinate discharge planning for short-term residents. The role requires maximizing reimbursement while ensuring Medicare compliance and OBRA adherence.

The successful candidate will oversee interdisciplinary assessments, develop concise care plans, and guide nursing staff to implement plans that support residents' highest practicable well-being.

Qualifications

• Must hold a current state RN/LPN license and be in good standing.
• Experience with OBRA and Medicare guidelines or MDS processes.
• Ability to coordinate interdisciplinary care plans and meetings.

Responsibilities

• Lead the MDS department and coordinate discharge planning for residents.
• Assess resident needs, develop and revise comprehensive care plans.
• Work with interdisciplinary team to ensure reimbursement and compliance.
• Conduct weekly care plan meetings per OBRA and state requirements.
• Review and update nursing plans to maximize quality of care and reimbursement.
