Job summary

Apple Rehab Uncasville is seeking an RN MDS Coordinator to lead the MDS department and coordinate discharge planning for residents. You will assess needs, develop concise care plans, and guide interdisciplinary teams to maximize reimbursement while ensuring Medicare compliance.

The role involves gathering information, presenting recommendations in written care plans, and conducting weekly care plan meetings per OBRA and state requirements. A current RN license and MDS experience are required.

Qualifications

• Must hold a current state RN license and be in good standing.
• Experience with MDS, OBRA compliance, and Medicare regulations preferred.
• Ability to develop and coordinate care plans with interdisciplinary teams.
• Strong organizational and leadership abilities to conduct weekly meetings.
