Apple Rehab Corporate in Avon, CT is seeking a skilled MDS Care Plan Coordinator to lead the MDS department, coordinate discharge planning, and assess short-term residents. The role focuses on maximizing reimbursement while maintaining Medicare compliance and regulatory standards.

The ideal candidate will manage comprehensive assessments with an interdisciplinary team, develop concise care plans, and ensure plans are reviewed and revised to meet residents’ highest practicable well-being.

Qualifications

• RN or LPN license with current state licensure.
• Experience with MDS, OBRA, and Medicare compliance preferred.
• Strong leadership and coordination skills across disciplines.

Responsibilities

• Lead the MDS department and coordinate discharge planning and assessments for short-term residents.
• Maximize reimbursement while ensuring Medicare compliance and regulatory adherence.
• Schedule and conduct weekly care plan meetings and ensure care plans reflect resident needs and goals.
