Job summary

Apple Rehab Inc. seeks a skilled MDS Care Plan Coordinator to lead the MDS department, coordinate discharge planning, and assess short-term residents.

The ideal candidate will maximize reimbursement while ensuring Medicare compliance and may manage short-term managed care cases. The role involves gathering data, assessing needs, setting goals, and developing organized, concise care plans with input from an interdisciplinary team.

Qualifications

• Must hold a current state RN or LPN license and be a nurse in good standing.
• Must meet all applicable federal and state licensure requirements.
• Attention to detail, good follow through skills and ability to prioritize multiple tasks.
• Ability to instruct others.
• Must be knowledgeable of general, rehabilitative and restorative nursing and medical practices, procedures, laws, regulations and guidelines governing long term care.

Responsibilities

• Gathers information, assesses needs, establishes reasonable goals, provides interventions and develops an organized, concise care plan.
• Coordinates completion of comprehensive assessment by interdisciplinary team and includes recommendations in the written care plan for each resident.
• Care plans must identify all relevant issues for the care of the resident as well as the goals to be accomplished for each problem or need identified.
• Works with the care planning team to implement final plans and encourage resident participation in development and review of care plans.
• Ensures nursing personnel are aware of the care plan and that plans are used in daily nursing services.
• Reviews nurses' notes and monitors to ensure care plans are followed and needs are met.
• Plans, schedules and conducts weekly care plan meetings per OBRA and state requirements.
• Completes the MDS with utmost accuracy and ensures highest level of reimbursement; compliant with CMS guidelines; Point Click Care experience is a plus.
