Job summary

Apple Rehab in Hartford, CT is seeking a qualified MDS Care Plan Coordinator to lead the MDS department and coordinate discharge planning for short-term residents. The ideal candidate holds a current RN or LPN license, understands OBRA and CMS Mega Rule requirements, and can maximize reimbursement while ensuring Medicare compliance.

Experience with Point Click Care is a plus; strong team collaboration is essential. You will work with an interdisciplinary team to review and finalize care plans.

Qualifications

• Must hold a current RN or LPN license and be in good standing.
• Strong attention to detail with ability to prioritize tasks.
• Ability to instruct others and work with interdisciplinary teams.

Responsibilities

• Lead the MDS department and coordinate comprehensive assessments by the interdisciplinary team.
• Coordinate discharge planning and optimize care plans per OBRA and CMS Mega Rule.
• Review and revise care plans, ensure reimbursement is maximized, and educate staff.
