Apple Rehab Uncasville seeks an RN MDS Coordinator to lead the MDS department, coordinate discharge planning, and assess residents. The ideal candidate will possess skills to maximize reimbursement as well as ensure Medicare compliance.

The RN MDS Coordinator may be responsible for managing short term managed care cases. The coordinator gathers information, assesses needs, develops care plans with the interdisciplinary team, and ensures plans reflect goals to reach the resident's highest

Qualifications

• Must hold a current state RN license and be a nurse in good standing.
• Experience with MDS (minimum data set) and Medicare compliance is required; candidates without MDS experience will not be considered.
• Ability to lead and coordinate an interdisciplinary care plan and OBRA/state guidelines.
• Attention to detail, prioritization, and ability to instruct others.

Responsibilities

• Leads the MDS department and coordinates discharge planning.
• Assesses short-term residents and maximizes reimbursement while ensuring compliance.
• Coordinates comprehensive assessment by interdisciplinary team and documents care plans.
• Schedules weekly care plan meetings and revises plans as required.
