Job summary

Parkville Care Center in Hartford, CT is seeking an experienced MDS Coordinator to supervise Medicare/Medicaid case-mix documentation, MDS completion, and care assessments to support reimbursement.

You will coordinate assessments with nursing, dietary, social services and rehab teams; facilitate care conferences; and ensure PPS/MDS updates are implemented with a focus on QA and regulatory readiness. Generous benefits and opportunities for cross-training.

Qualifications

• Active RN or LPN license in Connecticut is required.
• Knowledge of Quality Assurance / Improvement processes is required.
• Bachelor's or Associate degree in a health care related field preferred.
• Experience or training in MDS and PPS is preferred.

Responsibilities

• Coordinate the MDS process across care disciplines.
• Facilitate interdisciplinary care conferences.
• Complete and review MDS and PPS updates.
• Educate nursing staff on MDS requirements and sections.
• Respond to regulatory inquiries and maintain readiness.
