Walnut Creek Care Community in Kettering, OH is seeking an MDS Coordinator - LPN to oversee MDS assessments and care planning within regulatory guidelines, partnering with the interdisciplinary team to optimize resident outcomes.

The role requires an LPN license, 2–3 years nursing experience with long-term care, and strong data and EHR skills (PCC preferred). Part-time position with competitive hourly pay and benefits.

Qualifications

• Graduate of an accredited LPN program.
• Active and unrestricted LPN license.
• 2–3 years of clinical nursing experience, with at least 1 year in long-term care.
• Experience with MDS assessments preferred.

Responsibilities

• Coordinate and submit accurate MDS assessments in line with regulatory requirements.
• Collaborate with nursing, therapy, dietary and social services to implement care plans.
• Ensure timely MDS submissions to CMS and monitor for deficiencies.
• Serve as liaison with residents and families about MDS and care plans.
