Job summary

Freedom Village Bradenton is seeking a MDS Assistant with active CNA certification to provide clerical support and coordinate care plan activities in a skilled nursing setting.

You will liaise across nursing, therapy, business office, residents, and families, while promoting accurate point‑of‑care charting and documentation practices. Proficiency in EHRs and strong organizational skills are essential.

Qualifications

• Active CNA certification in good standing.
• Prior experience in skilled nursing or long-term care preferred.
• Familiarity with MDS/RAI, care planning, and EHR systems (Point-Click-Care, Matrixcare) preferred.
• Strong clerical/administrative skills: typing, filing, scheduling, data entry.
• Excellent organizational and written/verbal communication skills.

Responsibilities

• Provide general clerical support for the MDS office.
• Track and process managed care updates, authorizations, and notifications.
• Coordinate care plan meetings and calendars with residents and staff.
• Serve as the CNA documentation champion and monitor accuracy of at‑care documentation.
• Assist in onboarding CNAs on documentation systems and audits.
