Job summary

Apex-Healthcare is seeking an experienced MDS Coordinator to join our clinical leadership team in Florida. You will coordinate MDS assessments, ensure accurate documentation, and support reimbursement processes in a fast-paced long-term care setting.

The ideal candidate holds an active Florida RN license, has MDS experience, and demonstrates strong care planning, communication, and organizational skills within an interdisciplinary team.

Qualifications

• Active Florida RN license required.
• Prior MDS Coordinator experience in a skilled nursing facility required.
• Strong understanding of Medicare, Medicaid, and PPS processes.
• Experience with care planning and clinical documentation.
• Knowledge of PDPM and reimbursement systems.
• Strong organizational and communication skills.

Responsibilities

• Complete and coordinate MDS assessments accurately and timely
• Ensure compliance with federal and state regulations
• Collaborate with interdisciplinary teams on resident care plans
• Monitor documentation to support reimbursement accuracy
• Assist with audit preparation and regulatory compliance
• Support clinical and operational goals related to case mix and quality measures
