Job summary

Manchester Center for Rehabilitation and Healing seeks an experienced MDS Coordinator to lead MDS, CAAs and Care Plans for residents in a Skilled Nursing setting.

The role requires an active RN license, strong regulatory knowledge, and ability to work independently with a focus on high-quality resident care and outcomes. Collaboration with IDT and performance of RUG scoring are essential.

Qualifications

• Licensed Nurse with current, active license in state of practice.
• Minimum two years of clinical experience in a health care setting.
• Minimum one year of experience in a long term care setting.
• Prior experience as an MDS coordination accepted.
• Training program available for candidates with demonstrated assessment skills.

Responsibilities

• Oversees accurate completion of the MDS, CAAs and Care Plans in line with regulations.
• Acts as in-house Case Manager, communicating with residents, families and care teams.
• Communicates with Administrator and Director of Nursing to identify regulatory risk and resource needs.
• Collaborates with therapy staff to select the correct assessment reason and ARD; captures the RUG score.
• Understands MDS requirements for Medicare, Managed Care and Medicaid.
• Ensures timely electronic submission of all MDS to the state database and reviews validation reports.
• Leads Care Management with resident, IDT and family to resolve discharge barriers.
• Educates IDT and new staff on RAI processes and MDS procedures.
• Analyzes QI/QM data with DON to identify trends monthly.
• Responsible for timely Utilization Review and Triple Check processes.
• Serves on committees as required by supervision and Administrator.
