Job summary

Regency Integrated Health Services seeks a Care Management Specialist to coordinate the RAI/MDS process and ensure accurate resident assessments. You will collaborate with the interdisciplinary team to report and optimize ARD timing and reimbursement.

The role requires RN licensure with strong MDS/RAI knowledge, excellent documentation, and ability to lead care planning in a dynamic SNF environment. CAD and CMS-compliant reporting are essential.

Qualifications

• Must have strong knowledge of MDS/RAI processes and documentation.
• Experience coordinating interdisciplinary care teams.
• Ability to interpret clinical data for RUG classifications.

Responsibilities

• Coordinate the RAI process to ensure accurate and timely assessments.
• Ensure documentation supports services and reimbursement.
• Collaborate with interdisciplinary team to optimize ARD and care plans.
• Maintain schedule and track all MDS assessments (ARDs, PPS, annuals).
• Audit MDS processes and ensure CMS regulations are followed.
