Job summary

Acts Retirement-Life Communities is seeking an experienced MDS/Care Coordinator for our skilled nursing neighborhood. You will coordinate care for residents and new admissions, perform MDS assessments, and oversee the care planning process to ensure quality and compliance.

As liaison among the community, payer, and corporate office, you will oversee restorative nursing goals and staff education. Requirements include RN license, 2+ years of MDS experience in long-term care, and CPR certification.

Qualifications

• RN license is required.
• Minimum of two years' MDS experience in a long-term care setting.
• Current CPR certification or eligibility.

Responsibilities

• Coordinate care for residents and new admissions including MDS processing.
• Ensure timely and accurate completion of the MDS and care plans.
• Liaise between the community, payer, and corporate office.
• Oversee restorative nursing goals and staff education.
